Have tried but not doing to well. Would be very grateful for help. If I have made mistakes please correct them.  ???

The word before the date might be:  anted(i)c(t)o = beforesaid

The second word in the first line is:  cum = with

At the end of the second line:  Et comissa fuit

At the end of the third line:  Ex(ecuto)ri in eod(em) no(m)i(n)ato

The word before Ex(ecuto)ri looks to be a form of unius  but I'm not sure exactly what it is.

In the last line:  prius jurato

I'm not sure about the full form of the words not mentioned.
