Help with abbreviations R.N. record

  The words on this clip are from the record of a man in the Royal Navy. I am hoping someone can decipher and explain them. The entries for the village (Goodnestone) on Roll-of-honour have no details for him and I am hoping to get some added. He was invalided out and died in 1920.

"M.O.P." is presumably Ministry of Pensions

  I am also puzzled by the words on the left, under "invalided". Possibly something medical rather than military.

"Excision of tentacle" most commonly refers to the surgical removal of tentacle-like projections, often associated with tumors or other tissue growths.
