The WERTHEN surname doesn’t seem to turn up until Otto James WERTHEN’s Oct 1905 (first) marriage in (Anglican) St Barnabas church, Cape Town (
https://www.familysearch.org/ark:/61903/3:1:33SQ-GTWR-WYZ?i=34). Note: Family Search has various easy-to-find events for him after this 1905 marriage, so I won’t bore you with them.
Seemingly no Cape Town WERTHENs exist before 1905, so I had to assume that his surname must have altered in some way.
One thing that I did notice, was a
Dob = 7 May 1882 mentioned in his 1962 death info (

Eventually and with a bit creative searching, the birth/baptism of a
James Otto Henry WERTHEIM turned up. He was born on
7 May 1883 i.e. exactly a year later than the DOB mentioned on Otto’s death info. I have attached his 1 July 1883 baptism in St. George's Cathedral, Cape Town.
His parents were Wilhelm August Theodor WERTHEIM (b abt 1853) and Mart(h)ina (Martha) Hendrina Adriana CRUYWAGEN (b abt 1853). The couple were married on 4 February 1878:

DEATH - WERTHEIM August Theodore (in his 29th year) (husband of Marthina WERTHEIM, born CRUYWAGEN) (son of Johan WERTHEIM, of Hamburg, Germany) - on 9th January 1884 at Cape Town”
On 15 Sep 1884 mother Martina will marry a James CLARK:
