Death in Chile - translation please

Ancestry has a copy of the death register entry for a man they've indexed as Guillermo Tomas Whichelow y Walker, here is a link to the image:

It's number 1569, the bottom one on the LH page.  Can someone translate it for me please?

and the informant also seems to have signed in the final col.
Possibly Wm H Rookes ? as he signed Wm H, rather than the Spanish version of G[uillermo] I assume his native language was English - not necessarily British though.

there is a birth reg for a William Henry Rookes  Q4 1862, Axminster 5b 17 who would 'fit' with his age of 38 on the document - though that could take a fair bit of checking to see if he also was in Chile in 1900.
