It is written Ellen with a very long flourish at the end.  it is written Ellen Mellor then maiden surname is noted as Whittington.  So unless she was married, she has given a clue to the father's surname  and as said another clue possibly later by the use of the name Ernest.

If she was normally resident in England she may have gone to Ireland to give birth. The birth was registered by the 'occupier' E M Joy possibly the person from Coombe Hospital charged with regsitration in the name of the Superintendent, as we might call them now.

ETA: the index to the Irish genealogy site gives the name unknown Mella.  So could be advisable to search on all variations

Was there an Ellen in this Whittington family?

ernest guy mellor. he appears on the 1921 census for england aged about 12 years old

Not 12. It says 8 years and 7 months. That ties in to the November 1912 birth.

hello matty boy, when i looked closely at the doc again both the christian name and the surname mellor both have a trailing r  im pretty sure the christian name is ellinor. and there was a daughter albeit with the name variation of eleanor. the father of the child was a ernest james mellor. occupation on birth record is decorator. he was married 1910 to a lily wade. on 1911 census they are seperated, on marriage cert he is a decorator. eleanor married in 1911 to joseph evelyn jarvis, she was 4 months pregnant. there is no sign of joseph after 1911. all these 4 people lived in a space of 5 miles. details for ernest guy mellor on 1921 census living with grandparents b ireland, mother living, fathers whearabouts unknown. eleanor is living with her brother harry whittington with daughter edna born 1911.  there was just over a year between edna jarvis and ernest guy mellor. i thought place of abode on birth reg was the famous graffton street as of the brilliant luke kelly, but not sure. im thinking that probably ernest james mellor probably went to ireland with eleanor because from the age of 2 she had been deaf and dumb bless her. thanks very much for your help. stuart.

 I'm confused.

So this certificate is incorrect in that her name given as Mellor is incorrect as she was not married to the father, Mellor.

Was her daughter also born in Ireland and where is she in 1921?

Sounds very sad also.
