Railway Employment Record Cautions and Fines

Could someone tell me what the word marked by the blue arrow says. This is on a GWR Railway Employment Record under Cautions and Fines.

Dated 1915 I can read "Holder of Medallion of St John Ambulance" but I cannot decipher the final word, nor does it make sense as to why this is in the cautions and fines section of the record.
