Is it the one where he says he is a Canadian national? that would make sense as he was in Toronto in 1969

That is the one.
It's a hard site to link to results.
Maybe a bit hazy as to when exactly he did arrive in Australia. New Zealand is mentioned. Theater (designer) is the occupation on the card you can see in there.
I think height was 5 foot 11 inches.

If a very well known fugitive was going to assume another person's identity, I would have thought Derek would make an odd choice! And what then happened to the real one?
Strange that he is open on the 1939 Register though.
