It is on the second page (for me) and the box has a number 8 and says :-
Certificate to be signed by the soldier on discharge.
I hereby acknowledge that I have received all my pay and allowances (including clothing allowance), and all just demands up to the present date, subject to the reservations of the claims noted on the 3rd page.

(When a soldier is absent through illness or any other cause, and it is not desirable to forward these proceedings to him for a signature, a manuscript copy should be sent for the man to sign, and when returned should be attached here.)
