What sort of document is Hans Info 2 ??

Hi Dave, a kind lady from My Heritage has sent it to me. I had understood it to be a German document, but it has been snipped from Polish Taufbuch (baptism records) for 1908 (it appears for the date 22.10.1908 and is continued to the right of 'Hans 1' - but I can't view the original). H
