This is possibly a good place to put a recommendation for a series of books published by Family History Books, Introducing Health Care in Times Past by Ian H Waller. The first book in the series is particularly relevant to this topic.

The way I understand things, is that doctors studied for a degree (hence the title of Doctor), whereas surgeons studied under a senior surgeon (hence why they are titled Mr).
