Not sure about all of this, but here's an attempt

19. 3. 40 She shows no appreciable medical change
1940 Apr 22 Special report: Dementia. She is dull, stupid, and refuses
to speak. She takes no interest in her surroundings is
very impulsive and faulty in habits. Moderate health.
June 12 Phy.  Ex[amination] ? acid Wt. 7 st (losing) heart regular but sounds faint.  Lungs
? (lots of single letters, no idea what they mean) (? reflex ?? ? back
of left ear, not ??
Dec 10 mental note: She remains absolutely silent, with face like a mask . Remains passive ? Or under? Taken to lavatory ?, but eats her own food. ?
1941 April 21 Special report: primary dementia. She is dull, stupid, lies in bed
staring in front of her and seldom speaks. She is wet and dirty
in habits and needs every supervision. Moderate health.
June 9  Phy Ex[amination] Wgt – 6 st -4 Urine normal Heart regular ?? Lungs ??
Right upper eyelid slightly swolen & blurred looking. Cause unknown ?
Has been loose occasionally during two days & was ?? yesterday No temperature
Rectal swab negative for dysentery ?
Oct 2 ? swab negative dysentery worse

June 9  Phy Ex[amination] Wgt – 6 st -4 Urine normal Heart regular not poor quality Lungs ??
Right upper eyelid slightly swolen & blurred looking. Cause unknown squiggle for a signature
Has been loose occasionally during two days & was ?? yesterday No temperature
Rectal swab negative for dysentery nbm (= nil by mouth)
Oct 2 throat swab negative dysentery worse

? Abd n.a.d  CVS n.a.d ?? plantar reflex ?? scar back
of left ear, not discharging now. Squiggle for signature.

a brief explanation:
n.a.d = no abnormality detected. Abd = abdomen, CVS = cardiovascular system. Plantar reflex is a reflex where your toes move when the sole of the foot is scratched.
