Kate, I think that sounds quite plausible that there was just a misunderstanding about maiden name. 
I can imagine it might have felt a bit stressful having to go into an 'official' environment and give information at that time and mistakes might have been more commonplace.  I think  many of the population were illiterate back then too and probably anything to do with officialdom might have felt more daunting experience for many.

I’m happy to read this, it was what I was thinking too.

Thank you all for your help and for taking the time to look up information for me. I’ve given mum lots to read through and she’s delighted. It’s also brought back many memories of her grandmother and great aunts.
