If your friend's father is able to sign his name, then the rest of the form can be filled out by family.

I worded the first line of my post in the way that I did because I was unsure whether the Army Medal Office would, with nothing but a name and possible regiment, would be able to issue medals that had not been claimed.

I am still unsure given Ruskie's posts.  I'd suggest a first email to see whether in your particular circumstances, they will be able to help initially.
