Edit to add - Hypo/ertensive . . . . might be better.

Yes, now I can see Hypo at the start of the word, but I can't see Hypotensive.

Whatever it was, he'd had it for 4 years  :-\

Nor can I really - I was guessing after the Hypo and thinking what it could conceivably be.  Very tricky writing isn't it.

It's probably the worst writing that I've seen for a long time. I've read through all the cirrhosis info but can't find any of the causes that look like these words.

Is this a Canadian/US death cert?

I can't see hepatic though  you could expect it to appear.

I have looked and looked but cannot see hepatic.

Think you have line 3 right though.

I agree Laennec's cirrhosis.

I can't read anything on the second line except ascites - and I think that belongs on the second line, not the third, as the writing is smaller than that on the 3rd line and sits above the line.

I think the third line starts, "Old cardiac ---" then who knows what?!

This is shockingly awful writing!

Anyway. Ascites means fluid in the abdominal cavity and you get that with chronic liver failure, so that would go with the cirrhosis. Portal obstruction may also refer to the liver failure - if you look at this

it explains more about this, and maybe that would then fit with the hyper-something on the second line?

How about “old cardiac plaques”  The Letter looks like a “p” as in hypo/hypertensive. 🤔
