The last line is portal obstruction.

Last word on the third line is ascites.

Could it be cardiac lesion. The loops from the next line are invading the word!

More like Hypertension  I think - though it looks more like Hypo doesn't it.

Yes, now I can see Hypo at the start of the word, but I can't see Hypotensive.
