Well that line interpretation two makes more sense anyhow - I was trying last night to think of words starting with 'calci' but my brain wasn't working too well. 

We were thinking the 'with ascites' went with line two . . . and I think that would make sense too.

I think 'Lesions' for old cardiac . . .  though it doesn't look like it;
 the Drs I showed it to suggested irregularities . . . but??  doesn't look like it does it.

It is a puzzler alright!    ;D
