I have got some of my information from "paid subscriptions" which have proved correct.
I contacted the Demark Shire & Albany Library for help.
It appears the person who lodged the burial application made the mistake. Funeral directors records show the name Elizabeth Prudence. The plaque is going to be changed for future reference.
It just goes to show not is all that it seems

Hi there,

May I mention that Ancestry has compiled its own index from publically available sources, and in doing so may well have introduced flaws in their own transcribed database. 

It seems that their transcribing team's database "Australia, Death Index, 1787-1985" includes the following information about Elizabeth Prudence ANDERSON's death...

Birth abt 1934
Age 33
Death Place Western Australia
Father Lewis M
Registered in 1967
Registration Place  Plantagenet, Western Australia
Reg No. 10/68

I am not sure why any commercial website would seek publically available sources and combine various death indexes, to then label them as though they are one Australia Death Index when in fact each jurisdiction within Australia has legislative power over their own BDM registers.   RChat's Australia Resources Board includes live link to the following thread and a live link to a NAA fact sheet.

In my experience it has been the funeral directors who made arrangements for burials including all the contact with the cemetery trustees. 

I wonder who is going to pay for the cost of the new headstone/plaque ::)
