Help with translation from Polish to English

I was hoping someone could assist with translating the attached extract of three family members from the Ledger listing in handwriting persons registered at the Polish consulate in Shanghai, 1934-1941. I'd be keen to know what all the columns descriptions are and then what is written for each family member.

Have you tried google translate? (for the headings at least which should be readable):

Here's an amateur attempt.
1. serial number     2. Date of entry on register    3. Name and surname
4. Occupation    5. Religion    6. Place and date of birth    7. Civil status
8. Actual place of residence in the country and ???.
9. Place of residence in the said consular district
10. Documents on which entry on the register was based
11. surname, name, year and place of birth of wife and children
12. end of validity of passport
13. remarks.

4 is pretty illegible and 10 would need a native reader to decipher.
6 Jadwiga's birth place looks like Władywostok which is a long way from Poland but not that far from Shanghai.  Mum's may be a place in Poland, Łucja's looks like "Bandż", maybe the Bund?
7 says spinster, widow, unmarried resp.
9 says Szanghaj 32 rue Molière and perhaps "room 7"??
11 gives cross references between the entries

Rutter, Sampson, Swinerd, Head, Redman in Kent.  Others in Cheshire, Manchester, Glos/War/Worcs.
