The form used for the Leeds baptisms at the time doesn't give the mother's maiden name, and puts the details in columns - child's first name, parents first names, surname. There can be some ambiguity about whether the mother shared the surname.

I think the burial at St Marks Woodhouse is the most likely explanation for Emily Rosina's disappearance.
