Hi

It's just called Chisworth Methodist Church. It's a tiny church really, as Chisworth it self is very small. It's only 3 miles up the road from where I live. Hole House from what I can remember is a row of about 2 or 3 houses. Just further down on the opposite side of the church. A lot of my family came from Chisworth and I have Dewsnaps and Booths from my family tree from Chisworth & Charlesworth. Although a lot of the population of Chisworth seemed to be called Booth.

'Top Chapel' in Charlesworth was frequently used by families for christenings, Marriages and Burials.

A lady called Marjorie Ward seems to have done quite a bit of work on records, if you type her name and Charlesworth/Chisworth into google I'm sure you will be able to find information.
