It does not appear in the "  Parish Registers of Wales " .  I hope someone else can be of further help to you, and perhaps recognises it from somewhere .

i think its near Llannerch-y-medd so i would assume it comes under the Llannerch-y-medd parish

correction to the above its in the parish of LLANFIGAEL
