It's looking like St. John's ambulance then.  Thanks everyone.  Still don't know if it's my grandfather or his brother but I'm a bit further on!

In case anyone is interested, I now have confirmation that this is indeed my grandfather wearing his St. John ambulance uniform.  Many thanks for your help.  ;D
