I think I did, the reference number should just be for that person. I didn't fill in anything else.
Have you received an acknowledgement about the estimate? You are not obliged to accept it when you get it (should take about a week/10 days)
You can always ignore that & order again.

I doubt they will read all the paperwork to answer your specific questions, so you may get everything anyway.

Oh that's good. I have recieved acknowledgement of my request. Now that I know that it would contain more than one page, and I know roughly how much one page would cost, hopefully I should be able to tell from the estimate how many pages they'll be sending me. Just got to wait 10 days now!

I've just checked my last order, March this year, for cousin Lionel's war record. I imagine the copying charges are much the same.
It said all pages for that reference will be copied, at a cost of £20.70 including post & packing.
For this I got 46 pages Very good value.
