Death Certificate

Could someone please tell me what information is on a death certificate as i have never seen one before and need to know what info it has.

If this cert is not as big as you would like let me know.This Certificate is relitive to the Welsh region. I believe that English and Welsh Certificates house the same information. Scotland is different.

the following are given before 1969

when & where died
name & surname
gender & age
occupation
cause of death
signature, description & residence of informant
when registered
signature of registrar

After 1969 the following are also given

maiden surname of married women
date & place of birth
usual address of deceased
