Treeve,

I have a photograph of a Roll of Honour at Grimsby's St. James Church with the name Charles Forward, a naval rating,  upon it, also the name of a Charles Forward killed on HMS Lynx August 1918 on a Roll of Honour within a large book called a Kalendar Roll of Honour.

Both of these I can send to you should you wish it. However I suppose that they could refer to different Charles Forward.
