I think by 1966 there would be very few Churches offering burials, except perhaps for ones with a family plot. Durham GRO have very few Municipal Cemetery records, but I agree their services and staff are exceptional.
I believe that where a death was for a patient in a hospital unit attached to a Workhouse or Asylum (Mental Hospital), then the certificate would have usual place of residence noted if the patient was not an inmate, and it would have a similar noting in the burial/cremation record.
