It is a little unusual to have two addresses. The first one was under the date and I take it to refer to the address of the dead person, the second is in the box for the person who was present at the death.
I don't know that this Ellen Donohue is related in any way to the Jackmans, but I got the certificates just in case. I have an Ellen Donohue who lived in Wexford town, maiden name, Roche. Her sister was married to a Jackman/Chapman (not in town).
