Wondered why it says Shulver 'late' then the next enter says Shulver 'late Lucy Cook' but the parents names are down are Thomas and Mary

Hi, can someone help with explaining what the attached means : I've got Thomas Shulver married to Lucy Cook and having several children. According to my records Lucy died in 1882 but why would it say Mary on the children baptism records ?? Lucy is listed on all the census records with Thomas until 1881.

I'm trying to find out about my great Uncle - John Reginald Skinner born 13th March 1921 and died on the 26th January 1969.

He served in the Navy during WW2 - I can only find 1 document but don't really understand what is means :
By his name is shows Rank : Sid   - Official No : D/LX 20171 - Ship : Cardiff  - 10th March 1941
