Excellent translation! Deciphered the last few words:

Splinting & C (Splinting and Casting)
As above, impacted (correct) base index and Bennets' fracture right splinting. Massage. Slight ankylosis of 5th (added) Metacarpal-phalangeal index oint. Dicharged at own request though contrary to medical advise.
ride (correct) "request" attached.
