Excellent translation! Deciphered the last few words:

Splinting & C (Splinting and Casting)
As above, impacted (correct) base index and Bennets' fracture right splinting. Massage. Slight ankylosis of 5th (added) Metacarpal-phalangeal index oint. Dicharged at own request though contrary to medical advise.
ride (correct) "request" attached.

Thank you so much for adding these surnames. I had ancestors by the name of "Tait" that had lived in Hamilton for decades. I am curious too see if the "Tait"s that you had found are my ancestors. May I please see the images that you had taken? Any assistance would be greatly appreciated!
