Title: Railway Employment Record Cautions and Fines

Could someone tell me what the word marked by the blue arrow says. This is on a GWR Railway Employment Record under Cautions and Fines.

Dated 1915 I can read "Holder of Medallion of St John Ambulance" but I cannot decipher the final word, nor does it make sense as to why this is in the cautions and fines section of the record.

Perhaps they used the records to note credits occasionally as well, in red.  (No fine noted.)
