Registered a relative's Death recently and surprised to see that there is no place for Cause of Death to be noted on the Certificate.

You do not indicate in which country you are referring to, presuming it is the UK, there is a large space number 8 for cause of dearth, directly beneath the address of the informant. I am not aware of any changes since the most recent death certificate I have seen dated 2022.

That refers to England & Wales only, other countries may vary
It seems to refer to the medical certificate of death which is issued by a hospital/doctor/coroner once the cause of death is determined.
Then that MCD has to be produced when the informant goes to the Registrar to register the death in the civil register.
The GRO (again E & W only) FAQ has this page (last updated Oct 2024)

The Death was registered in Cardiff in May 2024.
1. date and place of death.
2. name 3. sex 4. maiden name
5.date and place of birth
6.description and usual address
7.name and surname of informant
usual address
8.signature of informant.
No reference to cause of death.

Because of the extra space need for Welsh/English translations of the headings, death certificates in Wales are slightly different and go to a second page.
Space 9 ( the cause of death/achos marwolaeth) should be on the back, or on a 2nd sheet.
