Hi, can anyone help me decipher these notes?
Here's what I have so far:
First image
21.9.39 There is no change in this ? condition. She is ? depressed mute and dirty in her habits
21.10.39. She is ? mute with no interestss
4.11.39 Admitted to wd 16 annexe ?
21. 11. 39 She is dull. ? and ? will not give any account? of herself ? ? ?. Dirty habits. ? Health ?
20.1.40 She is dull, ? + ? ?. She can give no account of herself in ? ? ? ? ? ? ?. ? ?
24.1.40 ? behind? ? ? ? ? ? ? ? ? ?
22.2. 40 Dull ? is in bed ? clean ? no change

1940 Apr 22 Special report: Dementia. She is dull, stupid, and refuses to speak. She takes no interest in her surroundings is very impulsive? and faulty in her habits. Moderate health.
June 12 ? ? ? ? Wt. 7 st (losing) heart regular but sounds faint. Lungs ? (lots of single letters, no idea what they mean) (cannot make out remained of sentence)
Dec 10 mental note: She remains absolutely silent, with face ? a mark ? ? ? ? taken ? ? ? ? ? her own food. ?
1941 April 21 Special report: primary dementia. She is dull, stupid, lies in bed staring in front of her and seldom speaks. She is wet and dirty in habits and needs every supervision. Moderate health.
June 9 (after this i can only make out a few words)

I think we have most of the first image now, but still quite a bit of the Jan 1940 note missing.
The first image so far:
21 September 1939: There is no change in this patient’s condition. She is still depressed, mute, and dirty in her habits.
21 October 1939: She is still mute with no interests
4 November 1939: Admitted to Ward 16 Annexe.
21 November 1939: She is dull, stuporous and {?}. Will not give any account of herself or show any interest. Dirty habits, mod. health.
21 December 1939: She shows no appreciable mental change
20 January 1940: She is dull, stuporous and exceptionally [?]. She can give no account of herself in [?]. Her habits are faulty. Mod. health.
24 January 1940: Sinus behind left ear leading into middle ear from broken down mastoid. {?} Some discharge.
22 February 1940: Dull {demented?} is in bed. Fairly clean, shows no change.
Any help from anyone is greatly appreciated! :)

a brief explanation:
n.a.d = no abnormality detected. Abd = abdomen, CVS = cardiovascular system. Plantar reflex is a reflex where your toes move when the sole of the foot is scratched.
