				I have two more documents associated with this patient that I had forgotten to post before the case notes, but these are the last. (I promise!)
 
 The physical description notes are dreadfully written, I can only make out the first few notes, and almost nothing after that. The urine test results are much easier to read but contains medical jargon I'm not really aware of!
 
 I can't decipher much, but here's what I think at the moment:
 
 
 PHYSICAL DESCRITPION
 Height 5-1 Weight 9-11 Temperature 98F, bodily condition: fat?
 Scars: old abdominal scar
 [no idea about the rest, looks like knee joint on the second to last line, though.]
 URINE TEST RESULTS
 Reaction acid, [?]
 all [?]
 [?]

				In the first one I think it's Bodily Condition - Fair (though some of the details don't sound wonderful).
 
 In the lower part, I think I can see:
 Tongue - Furred    Teeth - Nil
 Abdomen - Very adipose
 Herniae - Nil
 
 And the last line:
 Pupils equal do not react to light

 higher up, Pulse - Regular   Arteries - arteriosclerotic
 
 The rest is a mystery to me.

 Nervous system:  Knee jerks, plantar response.  Flexor.
 Pupils equal do not react to light
 
 Your second part is a report on a urine sample.

 Albumin nil.  Sugar nil.
 Centrifugal deposit.  Nil abnormal.
