I am doing research for a friend who was a twin but has no idea what happened to the twin, her brother, his birth is registered at Lewisham hospital as was hers but the family never mentioned him. I cannot find a death record. This was on October 1946. The birth certificates do not mention that they were twins. I speculate he was disabled or put up for adoption but would be grateful for any thoughts on what else might have happened and how to find out?

Birth certificates don't mention that the person is a twin but will have the TIME of birth, not just the date. Your friend's should have this on it as should the other one registered in Lewisham with the same surname / mother's maiden name.
As it is possible that the twin is alive, I think you will be asked to remove his name from your post.
If there is no time, could it be a coincidence - cousins perhaps?

Normally if twins were born on tbe same day time would, or should be on the birth certs as far as I understand
My husband was told he was a twin born circa 1939, home birth but there is no registration, and he never found any other details but the fact your friends twin was registered he must have gone somewhere, adoption sounds, a possibility, if she has the birth cert it might be easy to find a death, but he could still be alive judging by his birth date

Have you thought about purchasing his birth certificate just so you know for definite he is her brother.
Has she done her DNA.
