Title: Hospital record

Hi

I'm needing a few ideas on this record from the Dreadnought hospital in Greenwich.

On the top record I can see that the patient refused treatment but cannot decipher the complaint.

I'm tempted that the first word is "bullet", but not wedded to that. Any thoughts??
