Title: Workhouse examination shorthand

Hoping someone can help decipher the handwriting and shorthand in the attached workhouse examination record

This is what I have so far:
Maddison  Anscomb Hannah 15 (admitted by order of Mr
Hicks the Magistrate) ........... nor done any ............ Father was
born at Mayfield near Tunbridge Wells Sussex and has received
relief of ....... was born in the Stockhouse there

Along the left hand side:
her sister Elizabeth Bucklow  ..... Street

her fathers sister Eliza Green lives at No. 28 Bell Yard Carey Street ........

The sister's address is in Vine St.  The number could be 6 or 5.

Ser(van)t (I think) followed by:

nor done any act - means she hasn't done an act that would gain her a settlement in any parish.

Not sure of the shorthand term following act (I suspect it relates to legitimacy of birth or otherwise).  Then:

Issue of Wm
M. by              his Wife dec(ease)d


Not Stockhouse - Workhouse.

I can't help with the "workhouse shorthand" such as follows Magistrate.

Abbreviations expanded:

... Magistrate) never Married, apprenticed (or) Yearly Servant, nor done any Act etc. Lawful Issue of Wm ...

ADDED - ... & hath received Relief of said Parish. This deponent was born ...

ADDED - in the margin, after Carey St, I think it is Had a Shop Opposite.

Maddison   Anscomb Hannah 15 (admitted by order of Mr Hicks the Magistrate) never Married, Apprenticed, Yearly Servant, nor done any Act. Lawful Issue of William   by   his wife deceased. Father was born at Mayfield near Tunbridge Wells Sussex and hath received Relief of said Parish. This deponent was born in the Workhouse there.
