Can you please help I have found a Passaic City birth record which

maybe part of my family but it has not name of the child only surname

can I order a birth cert and what inform would it have on it also

where can I order it for I found the information on the  LDS site.

Birthplace Passaic City
Father Henry Leach
Mother Gertude Jones

You can order a copy of it through the LDS church using this form:  http://www.familysearch.org/eng/search/RG/images/FReqMicrofilm.pdf   I wouldn't be surprised if the information that's been transcribed is all that's on there.
