Title: POW record in Japanese

I have a POW record card of an ancestor who died working on the Burma Railway. There are about 5 lines of Japanese in the "Other Information" section. Could anyone translate it for me?

While I research FEPOW history I'm not too hot at the japanese translations.

If you are happy to give some more details about your relative I do have a database of British deaths along the railway and I'd be happy to look him up for you.
